The heart shape didn't mean love before the 13th and 14 centuries... But as the idea of romantic love began to take shape during the medieval period, so did the symbolism. People thought of hearts as books of memory, and they believed that feelings for the beloved were written on your heart.
As for the shape, it appears to have come from an illustration for the Italian poem, "Documenti d'amore". Shortly after its publication, the scalloped heart began appearing in other works of visual art and in tapestries. About 150 years later, the tapestry "The Gift of the Heart," now at the Louvre, depicted a man holding a small red heart.
By that point, the heart had taken its shape and had come to mean love.
